Primary Care Networks (PCNs) will only succeed if they can both develop and then implement their vision. To do that well they will have to engage and then fully support whole practice teams as they come together, collaborate, share staff, share delivery linked to income, hold each other accountable and not fall out. This is where a compelling vision for what you want to achieve is essential.
At SCW we are supporting a number of Primary Care Networks and are working with them to explore and agree a clear vision of the purpose of the network, supported by robust governance arrangements to ensure that decisions can be made at pace and that processes are proportionate.
We are also working with PCNs to implement projects that will ‘stress test’ the new ways of working and deliver benefits to patient care.
This approach supports the ongoing development of your Network to deliver high quality patient outcomes with financially viable solutions, in order to both ensure the sustainability of General Practice and maximise emerging opportunities.
