Isle of Wight Health system leaders commissioned SCW to help them to prepare for a site visit from the NHS New Care Models Team.
The overall goal was to clarify and consolidate a system-wide vision for the Isle of Wight Primary and Acute Care Systems (PACS) model which provided thought leadership by encouraging system leaders to be bold and radical in their thinking. The work would form part of the 'My Life a Full Life' new model of care and the programme of work to transform health and care system on the Isle of Wight.
Working in close collaboration with our clients, we co-produced a vision and ambition for the new care model together with communities, NHS, local authority, voluntary and independent sector staff and system leaders. This work featured the development of a maturity framework to assess the progress made towards a successful new care model and the next steps required to move forward at scale and pace.
We conducted a series of system-wide workshops to develop an integrated programme combining IT, infrastructure, engagement, and financial plans that could bring about the new model of care. An options appraisal was also completed for the new provider organisation form required to deliver the model of care.
To finalise this work, we managed the design and delivery of a two-day site visit for the NHS New Care Models Team, including an engagement event with more than 100 stakeholders.
- Working as part of a blended team with KPMG, we added in-depth knowledge of the opportunities and challenges facing the health and care system.
- Our programme management team, with their track record in delivering NHS system-wide change and integration programmes, had credibility with all stakeholders and were quickly accepted into the My Life Team
- We built insight based on the outcomes that were important to local people, with a view to developing an overarching logic model for the My Life Programme.
