There are nine integrated PACS Vanguards which will join up GP, hospital, community and mental health services. The Vanguards needed to establish logic models as part of their plans and bids for funding.
SCW enabled the logic models to be developed and refined by identifying the key components of logic model ‘best practice’ and reviewing each logic model against a suite of best practice indicators .
SCW provided intensive support for Vanguards to develop their models and local metrics for evaluation. The support was tailored to meet local needs, whilst incorporating learning from other Vanguards.
We produced a best practice guide for logic models PACS Vanguards.
We reviewed initial logic models with each Vanguard on 1:1 basis to identify where there were gaps or further detail was needed. We then supported them to improve their logic models.
We developed a menu of metrics for integrated care based on common outcomes and objectives for PACS as a resource for Vanguards to evidence their logic models.
We ensured all Vanguards were able to deliver logic models to the required standard, as part of their 2016/17 plans
The logic model approach is new to many NHS organisations and Vanguards valued the expertise and experience from the SCW team and our partners PwC.
Our team in the Quality Observatory are experts, and shared their specialist knowledge in developing metrics and indicators for new models of care to help the PACS Vanguards make rapid progress and provide assurance and confidence of the quality of their indicators.
By working across a number of Vanguards, SCW were able to share learning and experience between PACS.
