In response to the Five Year Forward View and CCG transformation programmes, we developed an online interactive mapping tool which provides CCGs and partners with map-based information to understand the CCG or STP footprint area.
The tool provides an innovative and easy to use visualisation of services, travel accessibility, activity/patient flows and population demographics across the CCG/STP footprint area, to support the CCG in engagement with stakeholders and communities.
An interactive map-based analysis of population and health data, to inform the Five Year Forward View and delivery of Sustainability and Transformation Plans (STPs).
The tool includes information on: service provider sites, hospital activity/patient flows, geographical boundaries, travel access (drive-times) for the current state of services, and population demographics such as deprivation and ethnicity rates.
Rich and powerful visualisation of information also enables users to annotate the map with drawings and text and then print the map in a number of different formats.
CCG users, transformation and information staff were involved in defining the contents of the mapping tool.
