We are exhibiting at Confed19: The future starts here

In line with the key theme for this year, SCW thought leaders and partners will be taking to the stage in the System Integration Zone (FA3) across the two days, and sharing insight and experiences from our latest programmes.

The theme of this year’s Confed19 conference, being held 19-20 June 2019 in Manchester, is the future, with the event offering a unique experience for health and social care leaders to face shared challenges, source practical solutions, debate topical issues and expand their professional network.

In line with the key theme for this year, SCW thought leaders and partners will be taking to the stage in the System Integration Zone (FA3) across the two days, and sharing insight and experiences from our latest programmes covering developing sustainable systems, Primary Care Networks, digital enablement and integrating to deliver people-centred care. Wednesday 19 June

Theme: How do we deliver sustainable systems? Sharing insights

Time: 10:30am – 11:30am

SCW’s  Clinical Lead, Liam Williams , together with partners, Andy Whiting, Director, Rubicon Health Consulting and Paul Sly, Partner, Clarity Consulting, will share some of the leading projects on sustainability and host discussion on creating and delivering sustainability at a system level and improving efficiency and productivity. 

Refreshments will be provided. 

Theme: Digitally enabled care at scale – lessons for success 

Time: 12:30pm – 13:30pm

Showcasing the innovations in digitally enabled care and highlighting the patient and clinical benefits from digital tools. Jo Watts, Global Digital Exemplar Programme Manager, University Hospital Southampton NHS Foundation Trust and David O’Connor, Principal Consultant, OSRI Ltd, will detail the ‘My Maternity Record’ solution being deployed in Hampshire and the Isle of Wight. Jocelyn Palmer and Peter Cambouropoulos, SCW programme managers for ‘Connecting Care’ and ‘Care and Health Information Exchange (CHIE)’, share the latest on integrated digital care records, in a session chaired by Andy Payne, Partner – Digital Transformation Consulting, SCW’, the country’s foremost Personal Health Records architecture expert.

Thursday 20 June

Theme: Primary Care Networks – Making change happen

Time: 10:15am – 11:15am

Alison Westmacott, SCW’s Director of Primary Care, will lead a session sharing examples of developments in Primary Care Networks. Chetal Sheth (GP) and Anna Morton (Practice Manager) discuss how the use of the SCW care segmentation model and mapped data has enabled different conversations around the care needs of the population served by North Sarum PCN. The NHSE CSU collaborative will present examples of support provided to emerging PCNs illustrated by case studies from across England.

Theme: ‘Speaking the same language’ – Leading place based care model transformation

Time: 12:00pm – 13:00pm 

Cameron Kinsella, Aster Housing Foundation Director, explores ‘creating mutually beneficial partnerships’ together with colleagues from the health and care system in a session chaired by SCW’s Richard Smale. They will consider the need to ‘speak the same language’, how analytical tools and aligning data sources across the system facilitate this care model transformation, and how segmentation and co-ordinated, targeted support across the system is integral to delivering measurable improvements in health.
