SCW is delighted to announce that their integrated care record and interoperability programme has been shortlisted for the Connecting Services and Information Award at this year’s HSJ Awards, recognising their outstanding contribution to healthcare.
The judging panel, made up of a diverse range of highly influential and respected figures within the healthcare community, have shortlisted SCW despite the tough competition from hundreds of excellent applicants. SCW have been selected based on their ambition, visionary spirit and the demonstrable positive impact that their project has on patient and staff experiences within the health and social care sector.
Driving forward the effective use of digital technologies to revolutionise citizens and staff experiences, SCW is working in collaboration with over 50 stakeholder organisations to develop and deploy multiple care record and interoperability programmes. Covering a population of over 3 million across the south of England, these shared care records support the delivery of safer and faster care by providing the right information at the right time in the right place to the right person. This enables professionals to see a single electronic view of up-to-date case management information, supporting better quality care provision, reducing service cost and supporting better decision-making and commissioning.
The aim of the overarching interoperability programme is to empower professionals to provide safer, faster treatment through access to accurate, timely and relevant data by2:
making it easier for clinical staff to securely access the right data
increasing numbers of individuals
increasing the depth and complexity of the data held, giving a more complete clinical history for each patient
enabling social care to share / access health and care data
supporting innovations around particular care pathways (e.g. EoL, SMI)
