South, Central and West (SCW) is delighted to learn we have been shortlisted for the Enhancing Care by Sharing Data and Information category at the 2017 Health Service Journal (HSJ) Awards. This prestigious award recognises the delivery of our Connected Care programme which is providing integrated data solutions for 18 health and social care partners across Berkshire.


HSJawards2017The Connected Care programme is a portal for securely sharing important patient data between GPs, community providers, local authorities, out-of-hours providers, mental health services and local hospitals. This ‘joined up’ way of working contributes to more accurate and efficient care.
Working in close partnership with Microsoft, Graphnet and the University of Edinburgh, the programme in Berkshire sees the design of an interoperability solution which provides unique opportunities for health and social care providers to access a whole system approach to data analytics.

Not only is this announcement recognition of the achievements of Connected Care and the difference it makes to health and care professionals and patients, it is also illustrates the growth of digital services at the heart of our NHS.

Members of the Connected Care team will now produce presentations and be interviewed by a judging panel made up of senior and influential figures from the health sector. The awards ceremony will be held on 22 November at the Intercontinental 02 in London.
