Population health analytics provides critical information to support population health management planning, by predicting both individual healthcare journeys to deliver person-centred care, and the costs involved.
As the NHS implements the Long Term Plan and health and care systems develop into Integrated Care Systems (ICS), funds must be accurately directed to ensure the delivery of a sustainable healthcare system.
In order to improve population outcomes, the current state of population health needs to be understood. For ICS this involves both considering information at a system level and for those geographies within the system.
Population health analytics provides critical information to support population health management planning, by predicting both individual healthcare journeys to deliver person-centred care, and the costs involved.
The information gathered in the intelligence phase of Population Health Management (PHM) informs 'what' interventions are implemented to 'who' in the population and 'how' this will be measured.
We help with the following:
Understanding your population
Population Health Management involves intelligence-led planning and delivery of services, aligning services with population need in order to improve outcomes.
We aim to help you to answer the pressing questions about how your system is currently performing and provide you with the depth of analysis you need to be able to effectively manage your resources to deliver the services your populations need.
System analysis
Answers the questions: How is our current system performing in comparison to similar peer systems? Does our system deliver the ‘expected’ outcomes based on our population, case-mix and key improvement initiatives? Where is the biggest variation in care outcomes and delivery?
Understanding flow
Answers the questions: What does the current patient flow through all of our system look like? Where are our patients going and what routes are they taking?
Actionable insight
Answers the questions: Using the above analysis coupled with further deep-dive intensive analytics, including localised population profiles, where should we focus our efforts? What are the key actionable insights? What are the target priority patient groups we should consider targeting?
We can support you to answer these questions using these cutting-edge tools :
- Insights Population Analytics
- Deep-dive Analytics
- Digital Intelligence
- National PHM Dashboard
Opportunity analysis
Opportunity analysis can be undertaken to determine which areas of focus might best meet the current organisational and population requirements. An opportunity analysis uses the data to identify opportunities to improve the quality, efficiency or equity of the care being delivered. It might include:
- Detecting unwarranted variation within the system
- Duplication analysis
- Gap analysis
- Quadruple Fail analysis (e.g. which events are occurring in the population that are simultaneously high cost, low quality, represent a poor patient experience and lead to increased inequalities
Outputs from a needs assessment should be combined with opportunity analysis to produce areas of focus that are actionable to deliver improvements in defined population health outcomes.
The information gathered in this phase of Population Health Management (PHM) informs 'what' interventions are implemented to 'who' in the population and 'how' this will be measured.
